Complete the Potential Client Information Application or contact the Clinic staff, 859.547.5542, for an appointment to discuss your particular tax issue. If you have completed the application, print and bring the completed form with you.
Please note that you will not automatically become a client by completing/submitting this form. After receiving your completed form, the clinic will contact you to set up an initial consultation meeting. If we accept your case after the consultation meeting, you will receive a retainer agreement. This agreement will outline the terms of services we will provide. Your representation will begin after we receive the signed copy of your retainer agreement.

Due to the high-demand for our services, even if you meet our income guidelines, a number of additional factors are used to determine whether or not we are able to formally represent you.
